Registration times are: Monday to Friday 10am-12pm & 2pm-5.30pm
Documents needed are: Proof of address & photo ID & NHS card or previous GP's name and address
Registrations must be done in person
Registrations forms must be completed in full. To register with our practice you will be required to complete two registration forms: one is the standard NHS GP registration form (GMS1) and the second is a new patient health questionnaire.
